A Washington Kink Enthusiast Dies After Injecting His Genitals With Silicone And His Mom Believes His ‘Master’ Is The One To Blame

A 28-year-old man named Jack Chapman, otherwise known as Tank Hafertepen, passed away on October 15. While his death didn’t make news at the time, that was in part thanks to the public cause of death, which was attributed to an undiagnosed lung condition.

However, a coroner’s report, along with an investigative report from Buzzfeed News, is revealing that it was actually due to silicone injections in his genitals. The report explains that silicone broke off from the injection site, traveled to his lungs, and caused respiratory failure.

Hafertepen had moved to the west coast of the United States from Australia. He had been in an eight-year relationship with Dylan Hafertepen. The pair were involved in Seattle’s and San Francisco’s BDSM and kink scene and characterized their relationship within that paradigm as master and pup.

Tank’s mother, Linda Chapman, says the coroner’s report makes it clear that his death was due to silicone embolism.

“The world has lost one of its brightest lights. Our precious Tank Hafertepen, 28, passed away this week due to a previously undiagnosed lung ailment. He is survived by his partner, Dylan Hafertepen, his mother and brother in Australia, and his loving chosen family in Seattle. We are absolutely devastated as Tank ends his adventure with us and begins a new one.

“Tank was the kindest, gentlest soul I’ve ever known. His heart was filled with pure love, which he showered on those close to him. His enthusiasm for just about everything, and the relationships that meant so much to him, was endless. His was a life worth living, which makes its sudden end so much more tragic. As I’m sure anyone who reads this will agree, knowing him made you want to be a better person.

“Tank had a persistent cough for several months. Otherwise, he seemed fine, doing the things he loved — working out, caring for clients as part of LifelongAIDS Alliance, and spending time with his family. Until a week ago when he had trouble breathing and was admitted to the hospital with what we figured was a routine infection. It turned out to be much more. He fought very hard, seeming to get a tiny bit better day by day, until the terrible moment that we were told nothing more could be done.

“After helping people, Tank’s greatest passion was for his partner, Dylan. Together for over eight years, their love overcame challenges the size of continents, and persisted as an unbreakable bond. Tank’s devotion and love for his partner was absolute. Tank’s final words to Dylan were ‘Thank you for allowing me to serve you’.

“His coworkers at Lifelong tell me that Tank was highly regarded by all he encountered, and that he touched many lives there. They grieve along with us.

“Tank entrusted me to be executor of his estate, a duty that I never thought I would have to fulfill. I would give anything to have him with us for one more moment, one more second, one more day. He wasn’t just my friend, he was like my baby brother. I will miss him forever, as will we all.

“In lieu of flowers, Tank’s wishes are that you consider making a donation to Lifelong AIDS Alliance ( https://give.lifelong.org/give/45401/#!/donation/checkout ), a non-profit Washington organization that provides food, housing, and health services to clients living with serious illnesses including HIV and AIDS. He loved his time there and we hope his legacy with Lifelong lasts indefinitely.

“Tank requested that his remains be cremated and that no public memorial service be held.

“His family also requests that our privacy be respected during this extremely difficult time. We understand that gossip can run rampant in our community, but we urge you to respect the deceased instead of adding to it. We will also likely be slow to respond to messages. The pain of Tank’s passing is tremendous and we’re just trying to get by, day by day.”

Linda Chapman blames Dylan Hafertepen, alleging that she has seen a contract between her son and Dylan that outlined the parameters of their relationship. Under the alleged contract, Tank was required to change his last name from Chapman to Hafertepen. He also had to sign over his earnings and was required to wear a chain around his neck. Linda Chapman also believes that it was Dylan Hafertepen who encouraged body modification and, in turn, was the one who injected her son’s genitals.

Despite this information, the Seattle Police Department and King County Medical Examiner have reviewed all of the files relating to his death and there is currently no criminal investigation.

But complicating the situation even further, Buzzfeed reported:

“There are other concerns as well. Three weeks before his death, Tank drew up a will, a copy of which was reviewed by BuzzFeed News, that left everything he owned, including $200,000 held in an investment account from his late father, to Dylan. But Chapman maintains her son had long pledged to leave that money to his autistic brother. Chapman thinks Dylan had something to do with Tank breaking that promise, and she’s retained Seattle lawyer Catherine Clark to contest the will.”

Many have wondered about where boundaries of consent and abuse lie in a scenario such as this.

Others who knew the pair want the police to take action, and the hashtag #justiceforjack began to trend on Twitter.

And while some are trying to figure out what happened, many are simply missing Tank:
